I use a $30 plantronics headphone/mic combo. Construction is crap, but the frequency response is quite good. I'm on my second pair.

My onboard sound sucks balls so I can't use my mic ...so no headphones. I've gone through several pairs through stepping on them. About 10-15 bucks each.

Because having things on my ears annoys the fuck out of me ...so unless I'm talking on TS I don't bother with it.
