Is it possible for any item to drop in hell meph runs?

No. Meph cannot drop the last tier of Elite Items, including Sacred Armor, Hydra Bows, Aegis', Coronas (pretty sure)...

mephisto can drop good items like shako, eth. titans, eth. skullders etc.. but not the very high elite uniques.
don't underestimate the power of colorfull dreams where you can fly !
