It happens because they're lining up a trigonometric unit with a regular geometric one. This occurs when they join the two triangles at the beginning; they're using the grid to line them up, but the slope causes the loss of area of one square.

For some reason, if a post had multiple attachments, the attachments were being ignored. After I enabled thumbnails, all attachements are displayed regardless of type and number. I consider that an improvement. If it's really that important that it not be a thumbnail, either make your image smaller than the generous (for a thumbnail) 150x150 limit, or host it somewhere.
